How to Request Copies of your Medical Records

We make every attempt to protect your medical records so they are not shared with others for any reason other than to promote the best health care for our patients. We require authorization from the patient or legal guardian to disclose your protected health information. The health care record is the property of Mitchell County Regional Health Center and is maintained for the use of clinics, hospital and medical staff for the benefit of the patient.

Please mail in the completed Medical Records Release form to 620 N. 8th St. Osage, IA 50461, Attention Medical Records. You can also fax the release to 641-732-6108 or email to mlroi@mercyhealth.com.
